Affidavit for a Passport of a Minor Child by either parent (who are separated but not formally divorced)

Affidavit for Lady applicant for change of name after Marriage

Specimen Verification Certificate

Change in Name/Deed Poll/Sworn Affidavit

Verification Certificate for Tatkaal Passports

Fill the application form properly with your name, address, contact telephone numbers (if any), and details of fee paid.
If you submit fee by a Draft along with the application, then fill draft’s details in the application form. Drafts or Postal order payable to Regional passport officer of your area
Applications can be submitted at the office in your area or can be sent by post at your area’s address.
